What is a verification record

A verification record is a signed, content-addressed statement that a specific set of claims about an asset was checked, by a named publisher, under a specific judge model and prompt version pinned by hash, at a specific time.

It carries the artifact digest, the pipeline used, the list of claims with their entailment and state (verified, in_review, or cut), a summary, and a signature. Because the record pins the judge prompt by hash, any later blind spot in that judge defines an exact, queryable cohort that can be re-verified and corrected with public errata.

A verification record is labeled VERIFIED. It is a stronger promise than attribution, so verification publishers are reviewed. The registry stores and serves the record; it never re-runs the judge and never re-scores. It reports what was verified; it does not adjudicate reality.

Resolve one at /r/{digest}. See also provenance record and judge hash.
